Important Form 1095-C Information
Form 1095-C, Employer-Provided Health Insurance Offer and Coverage, provides documentation of health insurance coverage offered to eligible employees. Employers are no longer required to automatically provide this form unless an employee specifically requests it.
PTSC will not be mailing Form 1095-C to employees. If you would like a copy of your form, please contact:
